Duodenal rupture complicating childhood non-accidental injury.

چکیده
Duodeno-jejunal flexure injury is well recognised in blunt abdominal trauma generally. It is usually caused by compression of the relatively fixed bowel against the spine producing a shearing force which tears the bowel.1 Non-accidental injury is a rare cause of duodeno-jejunal flexure injury in childhood, but serious complications can ensue if there is a delay in diagnosis, which unfortunately is common with this form of injury.

Duodenal perforation: a diagnostic pitfall in non-accidental injury.
Duodenal perforation is a rare, life threatening injury associated with non-accidental blunt abdominal trauma. Diagnostic delay is common, as the true history is concealed and signs may be minimal. Double contrast computed tomography is the most sensitive investigation to confirm clinical suspicion. We report three cases, all with other features typical of non-accidental injury.
متن کاملRupture of the spleen in a patient with a perforated duodenal ulcer and infectious mononucleosis.
A 72-year-old patient presented as a sealed perforated duodenal ulcer. This was later confirmed at operation when in addition a haemoperitoneum due to a lacerated friable spleen was discovered. The patient denied antecedent injury. The peripheral blood film was normal but a subsequent differential slide test for infectious mononucleosis was positive. Histology of the spleen showed hyperplasia o...
متن کاملTraumatic Duodenal Rupture and Avulsion of the Ampulla of Vater
Duodenal injury following blunt abdominal trauma is uncommon. The severity of injury can vary from an intramural haematoma to a duodenal rupture with associated transection of the pancreatic duct. A case of duodenal rupture with avulsion of the ampulla of Vater is presented and discussed.
متن کاملIntracranial haemorrhage and non-accidental injury.
Spontaneous subarachnoid haemorrhage in childhood is very rare and unlikely to be confused with traumatic haemorrhage. In children with spontaneous subarachnoid haemorrhage one third will be shown to have a ruptured aneurysm, one third will have an arteriovenous malformation, and the remainder will have no demonstrable lesion. Three large series reporting on over 10 000 patients with subarachno...
متن کاملSpontaneous oesophageal rupture with duodenal atresia in a newborn infant.
A newborn infant with spontaneous oesophageal rupture associated with duodenal atresia is described. The diagnosis was established by the pattern of hydropneumothorax on the chest x-ray film and by the aspiration of gastric juice by thoracocentesis. Necropsy showed a tear, which extended from the oesophagogastric junction to the upper portion of the stomach.
